How to start a baby oral care routine at 3 to 6 months
Reassuring guidance from one of our MamaSmiles dentists who sees this every day

At around 3 to 6 months, your baby’s mouth begins to change. Their gums feel different. Their behaviour shifts. They might chew more, fuss more, or suddenly dislike feeding. It’s the stage where teething begins, but it’s also the time when gentle oral care habits can start to form. These early steps don’t need to be perfect — they just need to feel calm and familiar.

We’re John and Jack, practising dentists and co-founders of MamaSmiles, and we see every week how much easier brushing becomes later when parents start small, steady habits at this stage.

Why this stage matters for future oral health

Even before the first tooth appears, your baby’s mouth is developing quickly. The gums soften, tissues change, and your baby becomes more aware of new sensations. Small daily habits help your baby feel:

  • comfortable with their mouth being touched
  • used to gentle routines
  • more confident when brushing begins

Years later, we see the difference. Babies who start calmly at this stage almost always find brushing easier as toddlers.

Simple oral care habits to start now

Let your baby chew safely
Cool teething rings, soft cloths, and baby-safe chew toys help relieve pressure and build comfort with mouth sensations.

Gently wipe the gums
Use a clean, damp cloth once a day. This keeps the mouth clean and gets your baby used to gentle mouth care before brushing begins.

Follow their chewing instinct
Chewing helps babies understand what they’re feeling. Offering safe things to chew supports normal oral development.

When the first tooth appears

As soon as that first tiny tooth peeks through, you can begin brushing.

Here’s what we recommend in practice:

Use a very soft baby toothbrush
Let your baby hold it, chew it, or explore it. Familiarity matters more than technique.

Add a small smear of baby-safe toothpaste or MamaSmiles teething gel
Babies often dislike strong flavours, which is why we created a very mild first toothpaste at MamaSmiles, made with hydroxyapatite to support early teeth without overwhelming taste.

Keep it short and calm
A few seconds is enough at first. You’re building comfort, not aiming for perfect cleaning.

Make brushing feel normal, not a battle

A calm atmosphere matters more than the brushing itself.

Sit your baby on your lap
They feel secure and can see your face.

Let them hold the brush
Babies love to copy what they see.

Choose moments when they’re settled
Avoid brushing when they’re hungry or overtired.

Use little rituals
Songs, smiles, or talking softly help your baby connect brushing with comfort.

When brushing feels normal, everything that follows becomes easier.

When to see a dentist

Bring your baby along to your own dental check-ups. Even a quick look helps them become familiar with the environment. Aim for their first proper check-up around age one, or sooner if you notice something that seems unusual.

If something doesn’t feel right, or your baby seems unwell in a way you can’t explain, speak to your GP, health visitor or NHS 111 for advice.

Quick summary

  • Start simple oral care habits at 3-6 months
  • Wipe gums gently once a day
  • Offer safe chewing to support teething
  • Begin brushing as soon as the first tooth appears
  • Keep brushing calm, short, and consistent
  • Let your baby explore the toothbrush and routine at their own pace

Early oral care is not about getting everything right. It’s about helping your baby feel safe, curious, and comfortable with these new sensations. At MamaSmiles, our goal is to make these early months feel less overwhelming for you and your baby.
